print help needed

Having trouble printing out a form from the dmv. It is not printing out the complete form that I need. Is there a way I can print out the portion I am missing.
Here is the form I am trying to print:

I am missing portion on the 1st page from:

my medical status/condition, is true and correct to the best of my knowledge.

am loading the PDF directly in a browser and printing from there.

Try downloading the PDF File instead.. either by right clicking or using an option in the viewer that you are using... In either instance some only capture and print the viewable area and some data can be lost... So by downloading the full document and either loading it and printing to your machine oe by using an Editor and filling the form out and sending it back to the DMV will probably be what gets you to your goal.. :)

try to chose the page layout from A4 to letter and chose the option fit to printer margins
