Using Windows XP, with or without the registry hack to receive POSReady updates, is not recommended. Many vulnerabilities that are patched in modern Windows versions are patched much later in POSReady, sometimes not at all. As you mentioned, one also runs the risk of incompatibility issues down the line.

I recommend you update to Windows 7 or Windows 8.1. If your computer is incompatible with both of these operating systems, I suggest buying a new computer. If you can't buy a new computer, I would go ahead and implement the POSReady registry hack. I haven't heard any complaints of incompatibility issues arising from the hack yet, and having the security updates from POSReady is better than having no security updates. Regardless of what you end up doing, make sure you are using an update-to-date internet browser that receives automatic updates, and make sure you are using an up-to-date antivirus that receives automatic updates.

Edit: Alternatively, you could also install a lightweight Linux distribution as a Windows XP replacement.

I'd take this with a grain of salt.

Extended support for Windows Embedded POSReady 2009 ends on April 9, 2019. Unless Microsoft somehow blocks the registry hack, one can expect to receive security updates on Windows XP until April 9, 2019.

While things like common sense, safe browsing habits, having an up-to-date web browser (i.e. not IE8), having an up-to-date antivirus, etc. matter greatly, one is still significantly less secure using Windows XP, with or without the POSReady registry hack. There are vulnerabilities that have been patched in later versions of Windows that have yet to be patched in POSReady 2009. Windows XP is also missing various security techniques, including but not limited to ASLR.

I highly recommend you upgrade to Windows 7 or Windows 8.1 if you can. I have an old Windows XP from 2002 that I upgraded to Windows 7. I thought it would run slowly with only 512MB of RAM, but it runs just fine for things like basic web browsing and word processing.
